Cultivate Mental Health PC, a telemental health practice providing psychiatric medication management services exclusively by experienced, board-certified Psychiatric Mental Health Nurse Practitioners. We currently are accepting Medicaid (including CHP+) and private pay clients and will be expanding to accept other commercial insurance later this year.
Cultivate Mental Health provides accessible, convenient and private behavioral health services to people living in all areas of Colorado. We know that assisting your clients to locate high-quality and timely medication management services can be difficult, and we at Cultivate Mental Health are committed to making this possible. When you refer your clients to Cultivate Mental Health, they will receive timely, individualized care when and where it’s most convenient for them.
Our secure, flexible tele-health visits allow your clients to schedule expert psychiatric medication management services during daytime, evening, and weekend appointment hours from any location where cell phone service is available. Our mission is to remove common barriers to accessible mental health medical treatment so your clients can conveniently continue their journey towards healing and wellness.
At Cultivate Mental Health, we understand the benefits of collaboration between professionals for improving outcomes. Our psychiatric nurse practitioners and support staff care team work with you to improve access to the coordinated, comprehensive continuity of care your patients deserve. We share your goal of creating client-centered, personalized treatment plans that reflect the input of all members of the treatment team- client, therapist, and medical provider. We are committed to regular communication with you and to being available to meet for care conferences at your request. With their permission, we will keep you informed of your client’s treatment plan and progress by providing you with treatment notes via e-mail or fax within 48 hours of the time of their appointment.
Care providers at Cultivate Mental Health treat a wide variety of outpatient behavioral and mental health disorders. We accept clients from age 15 to older adults and tailor our care to each individual’s unique needs. Clients of all races, ethnicities, genders, sexual orientations and religions/belief systems are welcome and will receive the quality care they deserve.
